Senior Case Manager

We are seeking a highly skilled Senior Case Manager to lead our support program for individuals facing chronic homelessness or sleeping rough in Cairns.

Our program aims to provide a stable, long-term housing solution for these individuals and promote their independence and well-being.

Key Responsibilities:
  • Provide expert case management and coordination of services to support clients in achieving their goals.
  • Develop and implement individualized support plans, ensuring clients receive the necessary resources and assistance.
  • Collaborate with external agencies and organizations to ensure seamless service delivery and maximize client outcomes.
  • Maintain accurate records and reports, tracking client progress and identifying areas for improvement.
  • Foster a culture of innovation and continuous learning within the team, staying up-to-date with best practices and sector developments.
Requirements for Success:
  • Qualifications in Community Work, Social Work, Psychology, or Behavioral Sciences preferred.
  • Proven experience in a leadership role within the homelessness sector, with a strong track record of achievement.
  • Ability to develop and maintain complex relationships with clients, stakeholders, and partners.
  • Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Proficiency in Microsoft Office and computer literacy.
