Homeless Health Outreach Therapist

We are seeking an experienced Occupational Therapist to join our Homeless Health Outreach Team in a key role that involves delivering high-quality care and support services to individuals experiencing homelessness or unstable housing.
The ideal candidate will have a strong background in occupational therapy, excellent communication skills, and the ability to work collaboratively with multidisciplinary teams.
- Key Responsibilities:
- Deliver holistic OT assessments and interventions to meet the complex needs of clients
- Develop and implement effective case management plans to promote client stability and well-being
- Collaborate with community service providers to ensure seamless referrals and service delivery
- Bachelor of Occupational Therapy degree or equivalent qualification
- Registration with the relevant professional association (e.g. AHPRA)
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong analytical and problem-solving skills
