About the Role

As a Senior Community Care Professional, you will be responsible for providing high-quality care and support to our residents, while also collaborating with our multidisciplinary team to deliver holistic care plans.

Key Responsibilities
  • Deliver exceptional care and support to residents, promoting their independence, dignity, and well-being.
  • Collaborate with the Lifestyle Coordinator and care team to plan and implement activities, programs, and services that meet the needs and preferences of our residents.
  • Develop and maintain strong relationships with residents, families, and other stakeholders, ensuring open communication and effective partnerships.
  • Contribute to the development and implementation of care plans, working closely with the interdisciplinary team to ensure comprehensive and person-centered care.
  • Participate in ongoing education and training, staying up-to-date with best practices and industry developments to enhance your skills and knowledge.
Requirements
  • A Certificate IV in Ageing Support or equivalent qualification.
  • Previous experience in aged care, community care, or a related field.
  • Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and colleagues.
  • A positive and proactive attitude, with a commitment to delivering high-quality care and service.
  • An Australian driver's license and willingness to transport residents as required.
