As a Nurse Specialist in Elderly Care, you will play a vital role in providing high-quality care to our elderly patients.
- Main Responsibilities:
- Conduct comprehensive assessments of elderly patients to create individualized care plans.
- Administer medications, manage chronic illnesses, and provide treatments in accordance with physician orders.
- Monitor vital signs and assess changes in health status, documenting findings accurately.
- Facilitate activities of daily living (ADLs), ensuring patient comfort and dignity.
- Collaborate with multidisciplinary teams, including doctors, social workers, and physical therapists, to optimize patient care.
- Educate patients and families about health conditions, medication management, and lifestyle adjustments.
- Implement and promote fall prevention and safety protocols to enhance patient well-being.
- Essential Qualifications:
- Current Police Record Check and Working with Children Check.
- Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.
