Adult Community Care Team Member

We are seeking a skilled Social Worker to join our Adult Community Care Team in a full-time position. The role involves working autonomously and as part of a team, with access to high-quality clinical supervision.

The successful candidate will work within a multi-disciplinary team to provide treatment and support to adult clients experiencing mental health issues. You will address the biopsychosocial needs of individuals on a client-centered, recovery-based model, promoting individual self-efficacy, independence, and community integration.

This role will require you to provide direct psychosocial interventions to consumers, their families, and carers, while also offering discipline-specific support to Case Managers through targeted interventions at Atherton Road and/or secondary consultation where required.

The ideal candidate will possess excellent engagement, relationship, and therapeutic skills, along with resilience and strong work ethics. A Bachelor of Social Work or equivalent qualification is essential, and Accredited Social Worker status is desirable.
