Adult Care Specialist

Job Role:
As a Social Worker in Adult Mental Health, you will work with adults experiencing acute psychiatric or mental illness and/or drug and alcohol dependency problems.
- Provide comprehensive psychosocial support through assessments, counseling, discharge planning, and advocacy to individuals, families, and carers, ensuring person-centered care.
- Coordinate timely discharges by working closely with housing services, the NDIS, Centrelink, and other community services to support patients' transition back into community life.
- Collaborate with mental health teams and community service providers to develop and deliver integrated care plans that meet individual needs.
- Engage families in practice, including conducting family meetings and supporting carers as key partners in recovery.
- Supervise junior Social Workers and provide guidance to students, contributing to their professional development.
- Ensure client safety through risk-minimizing practices and uphold evidence-based service delivery aligned with AASW standards.
