The Royal Freemasons community is seeking a skilled Care Coordinator to join our team at Coppin Centre Residential Aged Care Facility in Melbourne.

Job Description

As a Care Coordinator, you will be responsible for providing high-quality clinical care to older adults. Your key responsibilities will include:

  • Undertaking or supervising others to undertake clinical care for individual residents as required, including complex wound care, pain management, and behaviour management.
  • Liaising with Residential care staff, medical practitioners, allied health professionals, hospitals, and specialist health practitioners to ensure residents receive optimal clinical care.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA).
  • Recent experience in caring for high-care and dementia residents.
  • Knowledge of wound care, palliative care, behaviour management, clinical assessments, and care plans.
