Alfred Health, a leading healthcare provider, seeks an experienced Geriatric Care Specialist to join its multidisciplinary team.

  • Part-time and full-time positions available (64-76 hours per fortnight)
  • Located at Caulfield Hospital

The Department of Older Adults provides specialized subacute care for seniors requiring further medical assessment, rehabilitation, and support with complex discharge planning.

We are seeking a skilled and compassionate Geriatric Care Specialist to collaborate with our team on Ward ACG—a dynamic unit delivering high-quality, patient-centered care to older adults with complex needs.

  • A 15-bed Geriatric Evaluation and Management service, focused on subacute rehabilitation for older adults following acute hospitalization
  • A 15-bed secure Delirium and Dementia unit, dedicated to supporting patients experiencing acute cognitive impairment, including behavioral and psychological symptoms of dementia or delirium

Patients on Ward ACG often present with complex medical, functional, cognitive, and psychosocial needs.

The Geriatric Care Specialist plays a key leadership role in supporting the growth and capability of the nursing workforce and improving the quality of care provision on the ward.
