As a coordinator, you will manage a portfolio of customers receiving in-home care and lead a team of care workers.
Key objectives include ensuring excellence in client service delivery through assessments, goal planning, and onboarding new clients; supporting and managing a team of care workers in the community; managing client escalations and complaints effectively; converting new client leads into active customers; and developing strong relationships with clients, families, and external service providers.
To succeed in this role, you will need experience working with Aboriginal and/or Torres Strait Islander communities; a driver's license and ability to work in the area; previous experience in healthcare, aged care, or community services; and excellent case management and client communication skills.
