The Aboriginal Health Pathways Clinician is a part-time (35 hours per week), permanent position, located at the Caringbah Community Health Centre. The position is open to multiple disciplines: Social Work, Psychology, Occupational Therapy, and Counselling. The position is part of the SESLHD Out-of-Home-Care (OOHC) Health Pathway Program. The SESLHD OOHC Health Pathway Program provides coordinated health assessments for children and young people aged 0 – 17 years in statutory OOHC to improve their health outcomes. The program aims to provide a culturally responsive, strength-based, trauma-informed, and person- and family-centred service. This position will be part of a multidisciplinary health care team providing quality assessment, review and care coordination and navigation services to all clients on the OOHC Health Pathway Program.
Where You'll Be Working
Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

What You'll Be Doing

This position will provide psychosocial assessments, care coordination and navigation to assist children, young people and their families to access health services and improve health and wellbeing outcomes. It will have a focus on children and young people living in Out-of-Home-Care, including Aboriginal children and young people. The SESLHD Out-of-Home Care (OOHC) Health Pathway Program provides coordinated health assessments for children and young people aged 0 – 17 years in statutory OOHC to improve their health outcomes. The program aims to provide a culturally responsive, strength-based, trauma-informed, and person- and family-centred service. This role will be part of a multidisciplinary health care team providing quality assessment, review and care coordination and navigation services to clients on the OOHC Health Pathway Program.
The position will also build and maintain connections with Aboriginal Community Controlled Organisations, Aboriginal community services and the local Aboriginal community.

This is an identified Aboriginal position (an applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977 NSW). Qualifications in Social Work in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award, which provides eligibility for membership to Australian Association of Social Workers; OR Qualifications in Psychology with registration as a Psychologist with the Psychology Board of Australia; OR tertiary Counselling qualifications with eligibility for appropriate association membership; OR Occupational Therapy qualification and registration with AHPRA.
Demonstrated core competency skills in culturally informed assessment and care coordination and navigation within an Out of Home Care or trauma setting. This includes understanding of the impacts of trauma on individual health, development and wellbeing across childhood and adolescence.
Demonstrated high level written, verbal and interpersonal skills, to both engage clients and work effectively with the multi-disciplinary team.
Demonstrated understanding of challenges experienced by Aboriginal children and young people in accessing healthcare services.
Demonstrated commitment to the use and application of professional and clinical digital technologies.
Demonstrated participation or leadership in quality improvement processes and teaching, commensurate with years of experience.
Demonstrated commitment to clinical and cultural supervision and professional education and, if relevant to application, experience providing clinical supervision and leading education and teaching to staff and/or students.
Current NSW drivers licence (and a willingness to travel in accordance with the demands of the position).
