**Care Coordinator Role Overview**
We are seeking a skilled Care Coordinator to manage a portfolio of clients receiving In-Home care across the Bega region of NSW.
This role offers an opportunity to work with a diverse client base and make a positive impact on their lives.
The successful candidate will have a strong understanding of the needs of Aboriginal and/or Torres Strait Islander communities and be able to provide exceptional support to our clients.
Key Responsibilities:- Manage a portfolio of customers receiving In-Home care across the Bega region of NSW.
- Ensure excellence in client service delivery, including assessment of client care and goal planning.
- Develop exceptional relationships with existing and new clients and their families.
- Understanding of the needs of Aboriginal and/or Torres Strait Islander communities.
- Previous experience managing a service area or program within a Health Care, Aged Care or Community Service organisation.
- Australian drivers licence required.
- Vaccinated for COVID-19 (2 doses).
- Hourly rate of $43.47 + 11.5% Super.
