We are seeking an Aboriginal Health Worker to join our team in providing culturally responsive and client-centered care coordination services to Aboriginal families with children aged 0-5 within a Sustained Home Visiting model.

Key Responsibilities

The successful candidate will be responsible for:

  • Providing culturally responsive and compassionate nursing care to Aboriginal families.
  • Conducting home visits and offering practical health support based on primary health care principles.
  • Collaborating with Aboriginal Health Workers and allied health professionals to develop personalized care plans.
  • Educating families about early childhood development, health promotion, and parenting strategies.
  • Supporting referrals to community and government services to ensure a holistic approach to family well-being.
  • Advocating for Aboriginal health needs and contributing to the continuous improvement of services.
Requirements

To be successful in this role, the ideal candidate will have:

  • A Certificate III in Aboriginal & Torres Strait Islander Primary Health Care or relevant qualification.
  • Demonstrated experience delivering culturally responsive health services to Aboriginal communities.
  • Demonstrated experience working effectively as part of a multidisciplinary team to provide care coordination or case management services.
  • An Australian C-Class driver's license (including P1 or P2).
