Aboriginal Health Professional Opportunity

We are seeking a dedicated and compassionate Aboriginal Health Worker to join our team in this vital role. You will provide culturally safe, patient-centred care coordination to Aboriginal patients with chronic or complex health care needs.

Working in close partnership with clinicians, you will support patients and families through:

  • Providing comprehensive assessment of health and social needs, in collaboration with other health professionals
  • Providing care coordination to ensure seamless access to service
  • Providing education and advocacy to empower individuals and communities

This position is a unique opportunity to contribute to improving health outcomes and strengthening cultural safety within the healthcare system.

Key Responsibilities and Qualifications

The successful candidate will be responsible for providing culturally safe, patient centred care coordination services to Aboriginal patients and their families with chronic and/or complex needs.

The ideal candidate will have excellent communication and interpersonal skills, and be able to work collaboratively with a clinician to provide assessment, care coordination, and education to patients, families and carers that takes into account clinical, psychosocial and cultural needs.
