About Our Outreach Services

We are seeking a highly skilled and experienced professional to join our team as a Support Coordinator. This role will play a critical part in supporting young mothers during pregnancy and up to one year after birth, helping them develop the independence, confidence, and skills needed to break the cycle of homelessness and find long-term sustainable accommodation.

Key Responsibilities

  • Provide day-to-day operational coordination and leadership of our Horizon House accommodation locations and support programs.

  • Assist with the recruitment of caregivers and facilitate an induction to the role and program.

  • Facilitate regular team meetings, including case discussions and service coordination, and provide line management of team members.

  • Support a trauma-informed approach and embed it with all caregivers.

  • Work collaboratively with the Practice Lead to prepare and assess new referrals for presentation at Assessment Panel meetings.

  • Responsible for case management and the development and review of client plans with each program participant.

  • Develop strong networks within the local community and promote our services.

  • Support the State Manager to monitor and maintain resources within the allocated budget.

  • Keep and maintain records as required and ensure a high standard of reporting and documentation is maintained by team members.

Requirements

  • A well-developed understanding of trauma-informed care, including theoretical perspectives such as loss and grief, attachment, resilience, and connectedness.

  • A sound understanding of the challenges involved in service delivery to young people in the homelessness sector.

  • Experience in program coordination and leadership of a team to support program delivery.

  • A tertiary qualification in Youth Work, Social Work, or Psychology.

  • A minimum of 5 years' experience working with young people in a residential care setting.

  • Current and valid first aid and CPR certification.

  • A full, unrestricted driver's license.
