About the Role

Hearth Support Services is a leading provider of disability support services, and we are seeking a highly skilled and dedicated individual to join our team as a Disability Case Manager.

As a Disability Case Manager, you will play a critical role in developing and maintaining relationships with our clients, their families, and support workers. You will be responsible for assessing client needs, developing support plans, and coordinating services to ensure that our clients receive the highest level of care and support.

Key Responsibilities
  • Client Relationship Development: Build and maintain strong relationships with clients, their families, and support workers to ensure that their needs are met and their goals are achieved.
  • Needs Assessment and Support Planning: Conduct thorough needs assessments to identify client goals and develop personalized support plans that meet their needs.
  • Service Coordination: Coordinate services and supports to ensure that clients receive the highest level of care and support.
  • Communication and Collaboration: Communicate effectively with clients, their families, and support workers to ensure that everyone is informed and engaged in the client's care and support.
  • Case Management: Manage client cases to ensure that all necessary documentation is completed and that clients receive the support they need.
Requirements
  • Experience: Proven experience working in the disability sector, preferably in a case management or support coordination role.
  • Skills: Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, their families, and support workers.
  • Knowledge: Strong knowledge of the disability sector, including the National Disability Insurance Scheme (NDIS) and relevant legislation and policies.
  • Education: Relevant qualifications in a field such as social work, disability studies, or a related field.
