You will be a compassionate and skilled nurse who helps improve the lives of older adults living independently in the community.

You will use primary health care principles to deliver holistic, person-centred care to older adults experiencing mental health challenges, chronic conditions, and complex needs.

Working as part of a multidisciplinary team, you will provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

Key Responsibilities

Provide direct clinical nursing care to clients in their homes in line with best practice nursing procedures.

  • Conduct clinical assessments, dressings, medication assessment, and catheter changing.
  • Develop a holistic nursing care plan in collaboration with client, carer, Care Advisor, and Allied health where relevant.
  • Provide safe, quality nursing practice to clients with specific needs and/or complex needs (e.g. wound care, palliative care, education, and support).
  • Liaise with all parties involved in the care of the client to maintain holistic healthcare and wellbeing.
  • Ensure accurate and timely documentation.
  • Work safely to protect yourself, your colleagues, and clients while following all policies and procedures.
Requirements

To succeed in this role, you will need:

  • A genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar qualification.
  • Minimum 3 years post-graduate nursing experience.
  • Experience in clinical assessments.
  • Initiative and the ability to work independently.
  • Strong computer skills, especially Microsoft Office products, and the ability to learn new systems.
  • A NSW Drivers license and reliable vehicle.
