Duties and Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans that cater to each client's unique needs, ensuring timely documentation and service delivery aligned with best practice principles.
  • Mentor and train staff on clinical excellence, participate in competency frameworks, and offer ongoing guidance to enhance the quality of services delivered to older adults.
  • Collaborate with multidisciplinary teams to develop holistic care plans for clients with complex needs, advocate for client autonomy, and promote a high quality of life.
  • Establish and maintain strong relationships with community organizations, contribute to marketing efforts, and develop referral pathways to support service awareness and growth.
  • Ensure adherence to relevant legislation and quality standards, contribute to continuous service evaluation, and uphold high governance standards to deliver exceptional outcomes.
Requirements:
  • Tertiary qualification in Physiotherapy or related field with current registration is essential.
  • Valid First Aid certification (with annual CPR) and current driver's licence with a willingness to travel.
  • Strong knowledge of person-centred care and contemporary aged care practices.
  • Ability to mentor and support care staff, fostering skill development and improved client outcomes.
  • Excellent time management skills and commitment to service quality and business excellence.
