Advanced Nurse Practitioner Role

The Advanced Nurse Practitioner role at our organization presents a unique opportunity to make a direct impact on the health and well-being of patients. As a member of our team, you will have the chance to leverage your expertise and compassion to improve quality of care and identify chronic conditions that may otherwise go undiagnosed.


Key Responsibilities:
  • Conduct comprehensive assessments including medical history, diagnosis, and treatment, as well as provide health education, physician referrals, case management referrals, follow-up, and clear documentation according to established guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary to deliver patient-centered care.
  • Work closely with Primary Care Physicians (PCPs) on patient education and provide follow-up services to ensure optimal patient outcomes.
  • Provide healthcare services in various settings, including home visits and skilled facility visits.

Essential Qualifications and Skills:
  • Master's degree in nursing or equivalent experience and satisfactory completion of nurse practitioner licensure requirements.
  • Current RN and NP licensure in the state of practice, including prescription authority, or the ability to obtain prescriptive authority.
  • Board certification by a reputable professional organization, such as AANP, ANCC, or AACN, in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of one year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health record systems.
  • Excellent verbal and written communication skills for effective interaction with patients, clients, and colleagues.
