The role of RN Case Manager involves facilitating individualized patient plans in response to referrals from nursing, medical professionals and other healthcare team members.
This position requires collaboration with patients, families, physicians and interdisciplinary teams to ensure continuity of care throughout the healthcare continuum.
The RN Case Manager will lead efforts to achieve optimal clinical outcomes by coordinating healthcare services and ensuring that patients receive quality care as they transition between different levels of care.
Additionally, this professional will be responsible for overseeing utilization review activities for assigned patients, monitoring health care service utilization to maintain quality and cost-effective care in appropriate settings.
Key Responsibilities- Develop and implement individualized patient plans in response to referrals.
- Collaborate with patients, families, physicians and interdisciplinary teams to ensure continuity of care.
- Lead efforts to achieve optimal clinical outcomes through coordination of healthcare services.
- Oversee utilization review activities for assigned patients.
- Bachelor's degree in Nursing required; Master's degree preferred.
- Minimum 3 years of experience in case management or a related field.
- Strong communication and interpersonal skills.
