Job Summary

We are seeking an experienced Senior Case Manager to join our team in Wollongong. As a key member of our Services Team, you will be responsible for assessing client needs, developing care plans, and coordinating services to support individuals with complex needs.

Key Responsibilities
  • Conduct comprehensive assessments of client needs and develop individualized care plans
  • Coordinate and manage services to support clients with complex needs, including personal care, meal preparation, and domestic assistance
  • Work collaboratively with clients, carers, and families to ensure client-centered care and support
  • Manage staff performance and provide coaching and guidance to ensure high-quality care delivery
  • Develop and maintain relationships with community organizations and stakeholders to ensure a holistic approach to case management
  • Participate in business development activities to grow our client base and services
Requirements
  • Relevant case management experience supporting clients with high complex needs
  • Successful completion of a diploma or degree in Social Work, Social Welfare, Psychology, or a related discipline
  • Current driver's license and police check
  • First Aid and WWCC certifications
  • Excellent communication and interpersonal skills, with the ability to work with diverse stakeholders
  • Sound knowledge of current legislation, practice standards, policies, and procedures
