Advanced Healthcare Coordinator Position in Integrated Care

St Vincent's Hospital Melbourne is seeking an experienced Allied Health Professional or Nurse to join our Health Independence Program (HIP) Central Intake team as a HIP Central Clinician. This position plays a crucial role in managing referrals, identifying client needs, and ensuring accurate knowledge of eligibility criteria and current wait times.
Key Responsibilities:- Triage referrals based on clinical need, risk, urgency, and service capacity.
- Conduct initial needs identification and prioritization of new referrals according to HIP Guidelines.
- Advocate for clients as necessary and make referrals to other agencies when required.
- Provide education, support, and advice to referrers regarding HIP services, client eligibility, and alternative referral options.
- Offer allied health/nursing advice and support to the wider HIP Central team.
- Allied Health Grade 2 ($88,467 - $101,691 per annum, pro rata) or Registered Nurse Grade 3B YU11 ($102,278 per annum, pro rata)
- Allied Health or nursing qualification (Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) where required)
- Strong problem-solving skills, including understanding referrer requests and assessing client needs
- Highly developed clinical management and communication skills
- Working knowledge of the healthcare system and understanding of hospital-based community services and community-based programs
- Ability to work in a high-demand environment, prioritizing competing demands
- Current National criminal history check, or willingness to obtain one
