Job Title: Primary Healthcare Team Leader
Job Summary:
The Primary Healthcare Team Leader is responsible for overseeing the delivery of primary healthcare services within the organisation. This role involves leading a multidisciplinary team, ensuring high-quality patient care, and promoting health equity in the community.
Key Responsibilities:
- Team Leadership: Supervise and mentor primary health staff, fostering a collaborative environment that promotes professional growth and effective communication.
- Program Management: Develop, implement, and evaluate primary health programs and services, ensuring they are evidence-based and responsive to community health needs.
- Patient Advocacy: Serve as a key point of contact for patients, advocating for their healthcare needs and ensuring they receive comprehensive, coordinated care.
- Quality Improvement: Monitor and assess the quality of care provided, using data and feedback to identify areas for improvement and implement necessary changes.
- Collaboration: Work closely with healthcare providers, community organisations, and stakeholders to enhance service delivery and expand access to care.
- Education and Training: Provide ongoing training and support to team members on best practices, health promotion, and patient engagement strategies.
Qualifications:
- Education: Bachelor's degree in nursing, Public Health, or a related field; master's degree preferred.
- Experience: Minimum of 5 years of experience in primary healthcare, with prior leadership or supervisory experience strongly preferred.
