Peer Support Specialist

This role is ideal for an individual who has personal experience of substance use and recovery, with the ability to share their insights and support others in a similar situation.

The successful candidate will be responsible for delivering follow-up and outreach services to clients in various settings, including face-to-face interactions, telehealth sessions, and home visits. The primary goal is to provide person-centered care, facilitating the client's journey towards recovery and well-being.

This role involves working collaboratively with other healthcare professionals and teams to ensure a comprehensive approach to client care. Considerable training and supervision are provided to support the development of essential skills, including peer support, group facilitation, and case management.

  • Provide targeted follow-up and assertive outreach services to individuals in recovery
  • Support clients through assessment and intake processes
  • Refer clients to relevant services as needed, including support for carers and loved ones

We are seeking an individual with a strong foundation in AOD and mental health, preferably with tertiary qualifications in these fields. Essential skills include the ability to manage client caseloads independently and facilitate/co-facilitate groups effectively.

Key Responsibilities:

- Deliver follow-up and outreach services to clients in diverse settings

- Provide person-centered care and support to individuals in recovery

- Work collaboratively with multidisciplinary teams to ensure comprehensive client care

- Develop and implement effective case management strategies

Required Skills and Qualifications:

- Tertiary qualifications in AOD and mental health or related fields

- Personal experience of substance use and recovery, with the ability to share insights and support others

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team
