NDIS Support Coordinator

About the Role:

This role is focused on supporting individuals and families in navigating and achieving their NDIS plans and aspirations.

You will connect participants with appropriate NDIS plans and services, facilitating access to essential supports.

The successful candidate will manage varying levels of coordination and transitions for individuals up to 65 years old, addressing a wide range of complex needs.

Key Responsibilities include conducting initial assessments for new participants, coordinating with service providers to meet client objectives and needs, liaising with government agencies, community partners, and allied health professionals, and preparing regular Support Coordination reports for submission to the NDIA.

We are seeking someone with in-depth knowledge of NDIS regulations and reporting requirements, strong service delivery skills, and excellent time management abilities. A degree or qualification in Social Work, Human Services, or a related discipline, is highly regarded as well as minimum of 1 year experience in Support Coordination for individuals with complex needs.

Required Skills and Qualifications:

  • A degree or qualification in Social Work, Human Services, or a related discipline
  • A minimum of 1 year experience in Support Coordination for individuals with complex needs
  • Demonstrated understanding of NDIS regulations and prior experience in the NDIS sector
