Clinical Nurse Consultant Complex Care Job Description\

We are seeking a highly skilled and experienced Clinical Nurse Consultant to join our team in providing care coordination for older people with complex mental health needs.

\

The successful candidate will have extensive experience working with consumers and carers presenting with complex mental health issues, including physical comorbidities, psycho-social challenges, and addiction. They will also possess excellent education and guidance skills to provide internal and external partners with the necessary support.

Key responsibilities include:

  • Providing comprehensive mental health assessments and developing, implementing, and evaluating consumer-focused care plans based on evidence-based practice.\
  • Conducting risk assessments and developing strategies to mitigate risks associated with complex mental health needs.\
  • Working closely with multidisciplinary teams to ensure seamless delivery of care services.
\Required Skills and Qualifications\

To be successful in this role, you will need to possess:

  • A minimum of 5 years full-time post-graduate experience in mental health specialty.\
  • An approved post-graduate qualification within mental health, drug and alcohol, or other relevant qualifications/experience.
