About This Role
This position is an opportunity to work with clients who have complex health needs and provide care coordination within a dedicated multidisciplinary team.
- Assess and coordinate care for clients with complex health needs.
- Liaise with families and other services to facilitate drug and alcohol treatment.
- Provide clinical supervision, education sessions, and facilitate groups.
- Lead Quality Improvement projects.
- Communicate effectively with clients, families, and other stakeholders.

- Leading Quality Improvement projects to enhance client outcomes.
- Effective communication with clients, families, and stakeholders.
- Extensive post-graduate experience working with clients who have complex drug and alcohol issues.
- Demonstrated commitment to working with Aboriginal clients, families, and communities.
- Experience in working within a trauma-informed and harm reduction framework.
- Crisis intervention, short-term solution-focused intervention, complex assessment, and care planning experience.
- Excellent negotiation, decision-making, and advocacy skills.
