Care Coordinator for Complex Needs

This role involves working with individuals who have complex mental health needs, providing support and assistance to enable them to live independently in their own homes.
Key Responsibilities:- Provide individualized care and support to service users, promoting their independence and dignity.
- Monitor participants' health and well-being, contributing to the development of services that meet their needs.
- Experience working with people with disabilities, mental health conditions, and/or behavioural needs is highly regarded.
- Ability to communicate effectively with individuals from diverse backgrounds and provide culturally appropriate support.
- Excellent verbal and written communication skills, including ability to work collaboratively with colleagues and other stakeholders.
- Strong organisational and time management skills, with ability to prioritise tasks and manage competing demands.
- Current driver's licence and reliable motor vehicle.
- Second language skills are highly valued.
The successful candidate will possess a strong understanding of safe work practices, manual handling techniques, and the importance of maintaining professional boundaries. They will be able to work independently, report accurately to their supervisor, and contribute to the ongoing development of services.
Competencies required include:
• Ability to communicate sensitively and appropriately with people from diverse socio-economic and ethnic backgrounds.
• Familiarity with agency information management systems and policies and procedures.
• Ability to work in a fast-paced environment, prioritising tasks and managing competing demands.
