About the Opportunity
We are seeking a Youth Care Coordinator to join our team. This is an exciting opportunity to make a real difference in the lives of young people.
The primary responsibility of this role is to provide clinical care as part of the Check in Team based at Frontyard Youth Services, ensuring delivery of an efficient, effective, and clinically therapeutic service.
As a care coordinator, you will be responsible for delivering complex holistic health assessments and planning for person-centered client-driven goals, focusing on care coordination and improving self-management. This involves direct and indirect clinical care, maintaining an active caseload, and supporting clients to develop self-management skills in relation to their complex care needs.
Operational leadership onsite will sit with Frontyard.
About You
To be successful in this role, you will need to:
- Actively screen and engage with clients within the accommodation service and drop-in center.
- Manage a caseload of clients, providing support and brief interventions opportunistically with residents/clients who may have limited service engagement.
- Perform duties of the position to the best of your ability and to a standard acceptable to St Vincent's Health Australia.
- Take all necessary care and precautions when undertaking clinical procedures.
- Actively support consumers to make informed decisions about their treatment and ongoing care.
- Protect the health and safety of yourself and others, complying with all health and safety-related policies, procedures, and directions.

Requirements
To be eligible for this role, you must have:
- A relevant nursing or allied health qualification with minimum 5 years experience in a relevant acute, subacute, or community setting.
- Eligibility for registration/membership with AHPRA.
- Previous experience in a similar or related role, including working within a care coordination framework with people with complex psychosocial needs.
- Knowledge and experience in one or more of the areas of homelessness, mental health, substance use, young people with complex needs, and/or disability.
- Demonstrated understanding of issues impacting youth health and wellbeing.
- Proven ability to effectively manage and negotiate solutions to complex problems.
Screening Criteria
Successful applicants will require:
- National Police check.
- Working with Children check and NDIS Worker Screening check or ability to obtain.
- Valid Victorian Driver’s licence.
- Right to Work in Australia.
- Up to date with COVID-19 vaccinations or valid medical exemption.
