About the Role:

You will support children in care aged 14 and above, helping them prepare for adulthood and independence.

  • Assess their needs, develop tailored care plans, and coordinate services to support their education, health, and emotional well-being.
  • This includes creating Children Looked After (CLA) Plans, Education, Health and Care (EHC) Plans, and transition plans.
  • Work closely with families, carers, and partner agencies to ensure consistent support and advocate for the young person's best interests.
