Key Responsibilities:- Provide high-quality, evidence-based case management and clinical support to individuals and families impacted by Huntington's Disease
- Coordinate seamless care transitions between community, inpatient, and outpatient settings
- Develop and deliver educational programs and therapeutic interventions tailored to the needs of patients and their families
- Collaborate effectively with healthcare professionals and support services to ensure comprehensive, patient-centred care
- Current registration as a Registered Nurse with a reputable nursing authority
- Proven knowledge and understanding of the psychosocial and health needs of patients and their families
- Demonstrated expertise in mental health nursing, including assertive case management, triage, holistic physical and mental health assessment, clinical risk management, and crisis intervention within the community setting
