About the Role

We are seeking a highly skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.

Key Responsibilities:

  • Provide tailored person-centered services and activities to meet the unique needs and preferences of customers
  • Work collaboratively with customers, their families, carers, and community agencies to ensure seamless support
  • Develop and maintain strong relationships with customers, built on trust, empathy, and effective communication
  • Conduct regular assessments to identify customer needs and develop personalized support plans
  • Deliver domestic assistance services, including household chores, meal preparation, and personal care
  • Monitor and report on customer progress, identifying areas for improvement and implementing changes as needed
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and industry developments

Requirements:

  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues
  • Ability to work independently and as part of a team, with a strong focus on customer-centered care
  • Current Australian Drivers Licence with no restrictions, and a reliable and roadworthy motor vehicle for travel to customer homes
  • Comprehensive motor vehicle insurance and registration
  • NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment)
  • Current First Aid Certificate with 12 months or less currency (or willingness to apply)
