Support Professional

We are seeking a highly skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.
Key Responsibilities:
- Provide tailored person-centered services and activities to meet the unique needs and preferences of customers
- Work collaboratively with customers, their families, carers, and community agencies to ensure seamless support
- Develop and maintain strong relationships with customers, built on trust, empathy, and effective communication
- Conduct regular assessments to identify customer needs and develop personalized support plans
- Deliver domestic assistance services, including household chores, meal preparation, and personal care
- Monitor and report on customer progress, identifying areas for improvement and implementing changes as needed
- Participate in regular team meetings and training sessions to stay up-to-date on best practices and industry developments
Requirements:
- Certificate III Individual Support - Aged Care or Disability, or other relevant qualification
- Experience working with seniors, people on the mental health recovery journey, and those living with disability
- Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues
- Ability to work independently and as part of a team, with a strong focus on customer-centered care
- Current Australian Drivers Licence with no restrictions, and a reliable and roadworthy motor vehicle for travel to customer homes
- Comprehensive motor vehicle insurance and registration
- NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment)
- Current First Aid Certificate with 12 months or less currency (or willingness to apply)
