About the Role

We are seeking a skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.

Key Responsibilities
  • Provide tailored person-centered services and activities to meet the unique needs and preferences of customers
  • Work collaboratively with customers, their families, carers, and other professionals to ensure seamless support
  • Conduct regular assessments to identify customer needs and develop personalized support plans
  • Deliver domestic assistance services, including household chores, meal preparation, and personal care
  • Ensure customer safety and well-being by identifying and mitigating risks
  • Maintain accurate records and reports, adhering to Chorus' quality standards
Requirements
  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues
  • Ability to work independently and as part of a team, with a proactive approach to problem-solving
  • Current Australian Drivers Licence with no restrictions, and a reliable and roadworthy motor vehicle for travel
  • NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment)
  • Current First Aid Certificate with 12 months or less currency (or willingness to apply)

  • Attractive salary packaging options (up to $15,900 per year tax-free) and an additional $2,650 for Meals and Entertainment
  • Hourly rate of $31.04
