About the Role

We are seeking a highly skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.

Key Responsibilities
  • Provide tailored person-centered services and activities to meet the diverse needs of our customers
  • Work collaboratively with customers, their families, carers, and community agencies to ensure seamless support
  • Develop and maintain professional relationships with customers, their families, and other stakeholders
  • Contribute to a safe and respectful environment, upholding the principles of dignity and respect
  • Participate in ongoing training and professional development to enhance your skills and knowledge
Requirements
  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Current Australian Drivers Licence with no restrictions
  • Reliable and roadworthy motor vehicle to travel to customers' homes
  • Comprehensive motor vehicle insurance and registration
  • NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment)
  • Current First Aid Certificate with 12 months or less currency (or willingness to apply)
What We Offer
  • Attractive salary packaging options (up to $15,900 per year tax-free) and an additional $2,650 for Meals and Entertainment
  • Hourly rate of $31.04
  • Part-time permanent position
