About the Role

We are seeking a highly skilled and experienced Social Worker to join our team at Healthscope Hospitals. As a Social Worker in our acute care setting, you will be responsible for providing high-quality clinical care to patients and their families.

Key Responsibilities:

  • Provide assessment and evidence-based treatment to patients in acute care settings, including surgical, medical, critical care, emergency, maternity, paediatrics, and aged care.
  • Work collaboratively with Medical Officers and other healthcare professionals to develop and implement patient care plans.
  • Conduct social work assessments and provide counseling to patients and their families to address their social and emotional needs.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve patient outcomes.
  • Develop and maintain effective relationships with patients, families, and healthcare staff to ensure seamless care coordination.

Requirements:

  • Degree in Social Work and eligibility to the Australian Association of Social Workers (AASW).
  • Proven experience working in an acute healthcare facility.
  • Highly developed social work assessment and counseling skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Effective interpersonal, written, and verbal communication skills.
  • Ability to prioritize and manage workload demands, meet deadlines, and complete activities within scope of practice.
