We are seeking a dedicated early career Social Worker to join our dynamic team at the Central Adelaide Local Health Network. As a Social Worker, you will play a key role in providing high-quality Social Work services to patients and families, working collaboratively with a multi-disciplinary team to achieve the best possible outcomes.
Key Responsibilities:- Provide assessments and interventions to assist patients and families in coping with their situation.
- Collaborate with a multi-disciplinary team, actively participating in ward rounds and meetings.
- Work within both CALHN services and the community to ensure the best possible patient outcomes.
- Access and utilise community resources to support patient care across multiple locations.
- Participate in quality improvement projects and research to drive service excellence and best practices.
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
