About the Role

Able Australia is seeking a highly skilled and compassionate Support Coordinator to join our team. As a Support Coordinator, you will play a vital role in assisting National Disability Insurance Scheme (NDIS) and Disability Services Outreach Australia (DSOA) clients in the support coordination and implementation of all supports in their funded plan.

Key Responsibilities
  • Assist clients in the support coordination and implementation of all supports in their funded plan, including informal, mainstream, and community supports, as well as funded supports.
  • Work collaboratively with clients to understand their needs and goals, and develop personalized support plans to achieve their desired outcomes.
  • Develop and maintain relationships with clients, their families, and other stakeholders to ensure effective communication and coordination of support services.
  • Conduct regular reviews and assessments to ensure that clients' needs are being met and that their support plans are being implemented effectively.
  • Provide education and support to clients and their families on NDIS and DSOA policies, procedures, and services.
  • Collaborate with other service providers and stakeholders to ensure a coordinated and seamless delivery of support services.
Requirements
  • Minimum 2 years' experience as a Support Coordinator in the disability sector.
  • Strong understanding of support coordination practices and principles, including the NDIS Act 2013 and Disability Act 2006.
  • Excellent communication and interpersonal skills, with the ability to work with a diverse range of clients, including those with complex needs.
  • Ability to work autonomously and as part of a team, with a strong focus on collaboration and partnership.
  • Knowledge of disability support services, including informal, mainstream, and community supports.
Desirable Qualifications
  • Auslan skills, including Auslan as a first language or completed Auslan studies.
