Position Overview

This role is focused on supporting Aboriginal and Torres Strait Islander individuals with severe mental illness as they transition from custody to the community.

The successful applicant will be part of a multidisciplinary team that collaborates with custodial mental health services, primary health networks, and other organizations to ensure continuity of care.

Job Description:

Key Responsibilities:

  • Provide case management and discipline-specific skills to support Aboriginal individuals with complex healthcare needs
  • Develop mental health care plans and establish necessary therapeutic and cultural supports
  • Collaborate with the Custodial Mental Health Service, primary health networks, and other organizations to ensure continuity of care
Required Skills and Qualifications

To be successful in this role, you will need:

  • Be of Aboriginal or Torres Strait Islander descent
  • Bachelor degree or higher relative to the role
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) for Occupational Therapy or Psychology professions
  • Eligibility for membership of the Australian Association of Social Workers (AASW) for Social Work
  • For Nursing staff – Current unrestricted authority to practice as a registered nurse with the Nursing and Midwifery Board of Australia
