Job Title: Youth Transition Specialist

This role focuses on reducing re-incarceration by addressing complex issues such as drug and alcohol use, homelessness, social isolation, physical and mental health, education, employment, family relationships, financial hardships, and histories of trauma.

Duties:
  • Provide transitional support services for young people with multiple and complex needs.
  • Develop strategies to assist clients in reconnecting with family and culture.
  • Collaborate with families to assess suitability for returning home.
  • Establish working relationships with clients prior to release.
  • Assist clients in accessing education, employment, and pro-social activities.
Required Skills and Qualifications:
  • Identify as an Aboriginal or Torres Strait Islander person.
  • Experience in providing trauma-informed care.
  • Casework experience.
  • Knowledge of complex needs, including criminal justice system involvement, AOD, intellectual disability, and mental illness.
  • Demonstrated cultural competency and knowledge of Aboriginal and Torres Strait Islander issues.
  • A current NSW Driver Licence.
  • A current NSW Working With Children Check.
  • Criminal Record Check.
  • Fully vaccinated against COVID-19, including booster.
