Catholic Healthcare is seeking a skilled Aged Care Assessment Expert to join our dynamic team in New England. This role offers an exciting opportunity to make a real difference in the lives of older Australians by assessing their needs and ensuring they receive the right care.
Job SummaryThis Aged Care Assessment Expert position involves conducting assessments to identify client needs and referring them to appropriate home support services. The successful candidate will be responsible for working collaboratively with clients, families, and multidisciplinary teams, as well as reviewing support plans to meet changing client needs.
Responsibilities- Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
- Collaborate with clients, families, and multidisciplinary teams to promote a supportive and positive team culture.
- Review support plans to meet client's changing support needs.
- Work effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
- Australian Driver's license and access to a registered, insured vehicle.
- Excellent communication skills, both written and verbal.
- Commitment to excellence in customer service, both to internal and external stakeholders.
- Excellent planning, organising and problem-solving skills.
- Previous knowledge or work experience in the Aged Care System preferred.
- A competitive salary of $47.66 per hour, plus superannuation.
- 7 additional leave days per year.
- Pay less tax with Not For Profit Salary Packaging.
- Travel will be reimbursed at 0.88 cents per km in line with our travel policy.

- Paid Parental leave and Long Service Leave after 5 years.
The estimated annual salary for this position is approximately $98,323.20, based on a 38-hour workweek and assuming no overtime.
