Catholic Healthcare is seeking a skilled Aged Care Specialist to join its dynamic team in New England. As a Clinical Aged Care Assessor, you will play a critical role in assessing the needs of older Australians and ensuring they receive the right care.

About the Role

This is an exciting opportunity for a qualified health professional to make a real difference in the lives of older Australians. As a Clinical Aged Care Assessor, you will conduct assessments to identify the complex needs of clients and refer them to appropriate aged care services under the Aged Care Act.

  • Collaborate with clients, families, and multidisciplinary teams to provide comprehensive support
  • Work collaboratively with colleagues to promote a positive team culture and achieve key performance indicators
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
  • Review support plans to meet clients' changing support needs
  • Work effectively autonomously and as part of a broader team to deliver high-quality outcomes within identified timeframes
  • Maintain currency of knowledge relating to aged care services, both funded and non-funded
About You

To be successful in this role, you will require:

  • Tertiary qualification in a health-related discipline directly related to health, aged care, or a specialist area (e.g., Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist, or Social Worker)
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • An Australian driver's license and access to a registered, insured vehicle
  • Minimum one year of demonstrated experience in Australia or overseas delivering services in aged care settings and/or to older people, including those living with dementia
  • Willingness to apply for a Police Check
