Care Advocate for Aged Services

We are seeking a highly skilled and compassionate Care Advocate for Aged Services to join our dynamic team at Catholic Healthcare. This exciting opportunity will allow you to make a meaningful impact in the lives of older Australians, assessing their needs and ensuring they receive the right care.
Key Responsibilities- Conduct thorough assessments to identify client needs and refer them to appropriate home support services
- Collaborate with clients, families, and multidisciplinary teams to promote a supportive and positive team culture
- Work autonomously and as part of a broader team to deliver key performance indicators within identified timeframes
- Australian Driver's license and access to a registered, insured vehicle
- Excellent communication skills, both written and verbal
- Commitment to excellence in customer service, both internal and external stakeholders
- Previous knowledge or work experience in the Aged Care System preferred
- A competitive salary of $47.66 per hour + superannuation
- 7 additional leave days per year
- Pay less tax with Not For Profit Salary Packaging
- Travel reimbursement at 0.88 cents per km in line with our travel policy
