About the Role
This is an exciting opportunity for an experienced Registered Nurse or Enrolled Nurse to provide exceptional care coordination in our Home and Community Services division. Based in Tamworth, you will be responsible for managing a caseload of home care package clients, ensuring their needs are individually assessed and met in line with McLean Care's values and policies.
Your Key Responsibilities
- Deliver holistic nursing care to clients, including managing a caseload of home care package clients.
- Ensure client needs are individually assessed and met in line with McLean Care's values and policies.
- Work collaboratively with the Community Program Manager to develop and implement care plans that meet client needs.
- Promote a culture of safety and quality within the team, adhering to McLean Care's policies and procedures.
Requirements and Qualifications
- Current APHRA Registration as a Registered Nurse or Enrolled Nurse.
- Comprehensive clinical knowledge, particularly regarding age-related health issues.
- Previous experience in care planning and clinical assessment.
- Strong leadership skills and ability to support staff to deliver quality outcomes for clients.
- Excellent communication, interpersonal, and negotiation skills.
