Catholic Healthcare is expanding its Aged Care Assessment Services team, creating opportunities for highly skilled professionals to join our dynamic and supportive team.

We are seeking a Clinical Aged Care Assessor to conduct assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services under the Aged Care Act.

About the Role

  • This role involves conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • You will work collaboratively with clients, families, and multidisciplinary teams as required, promoting a supportive and positive team culture.
  • Maintain currency of knowledge relating to aged care services, both funded and non-funded, and assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.

What We Offer

  • A competitive salary of $54.47 per hour, plus superannuation, providing a total remuneration package.

Your Skills and Qualifications

  • Tertiary qualification in a health-related discipline directly related to Health, Aged Care or a related specialist area.
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • An Australian Driver's license and access to a registered, insured vehicle.
  • Minimum of one year demonstrated experience in Australia or Overseas delivering services in Aged Care settings and/or to Aged persons, including those living with dementia.
