Occupational Therapist Role

This role presents an exceptional opportunity for a dedicated Occupational Therapist to make a meaningful difference in the lives of clients with complex care needs. The successful candidate will be responsible for conducting comprehensive assessments, providing intervention and management, and promoting empowerment for clients to achieve optimal functional recovery and improve their health outcomes.

About the Position
  • As an Occupational Therapist, you will work closely with clients experiencing safety, functional, medical, and environmental issues that impact on their functional independence. You will conduct home assessments, prescribe equipment, falls prevention measures, pressure care solutions, and retraining in activities of daily living.
  • The position also involves providing person-centred care coordination for eligible, high-risk clients living with chronic disease, comorbidities, complex aged care, and other health and wellbeing needs. You will conduct comprehensive wholistic health assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other healthcare professionals, and ensuring appropriate linkages with hospital and community services.

Key Responsibilities:

  • Develop and implement individualized care plans to address clients' complex care needs.
  • Conduct thorough assessments of clients' physical and cognitive abilities, identifying areas for improvement and developing strategies to enhance their functional capacity.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery.
  • Provide education and support to clients and their families to empower them to manage their care and promote self-management.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.

Required Skills and Qualifications:

  • Bachelor of Occupational Therapy degree or equivalent qualification.
  • Minimum 3 years' experience in relevant acute/community setting.
  • Strong assessment skills and case management expertise.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Good knowledge of community supports available for clients.
  • Current Victorian Driver's Licence.
