**Job Title:** Disability Case Manager

Are you a skilled professional with experience in case management, looking for a challenging opportunity to utilize your skills and make a difference in the lives of vulnerable individuals?

This is an exciting role within our organization where you will be responsible for managing a caseload of complex cases, working closely with internal and external stakeholders to provide support and advice on various issues.

Key Responsibilities:

  • Undertake end-to-end case management of Administrative Appeals Tribunal (AAT) cases, ensuring that participants receive the necessary support and resources.
  • Maintain regular contact with internal and external stakeholders, providing technical, detailed, professional, policy, operational support, and advice on complex issues.
  • Attend case conferences, conciliations, and hearings conducted by the AAT, advocating for the best interests of participants.
  • Develop and implement participant plans to reflect funded supports, ensuring that participants' needs are met.
  • Prepare documents detailing resolution options and outcomes at case management meetings, keeping stakeholders informed.

Requirements:

  • Previous case management experience in the disability, legal, community services, or healthcare sector.
  • Excellent assessment, documentation, and care plan implementation skills.
  • Experience interpreting information and legislation to reach a sound agreed resolution.
  • Exceptional communication, problem-solving, negotiation, and conflict management skills across diverse stakeholders.
