Job Title: Community Case Management Manager

As a seasoned professional, you will oversee clinical, administrative, and supervisory aspects of the Core Case Management and Scattered Site teams (collectively, Community Case Management). This role involves working closely with program coordinators to ensure program goals and objectives are achieved within designated timeframes.

The ideal candidate possesses a Master's degree in social work or a related discipline, along with at least five years of experience in mental health, chemical dependency, or social services, including one year of supervisory experience. LSW / LPC is required; LISW / LPCC is strongly preferred.

Key Responsibilities:

  • Lead the community case management team to achieve program goals and objectives.
  • Develop and implement strategies to enhance program effectiveness.
  • Collaborate with program coordinators to ensure seamless day-to-day operations.

Required Qualifications:

  • Master's degree in social work or a related discipline.
  • At least five years of experience in mental health, chemical dependency, or social services.
  • One year of supervisory experience.

Benefits:
