Care Manager Position
This role requires a highly skilled and dedicated individual to manage the delivery of high-quality, person-centred care to participants. The successful candidate will be responsible for ensuring that services are consistent with relevant legislation, quality frameworks, and funding guidelines.
- Implementing and maintaining a person-centred care model to enhance individuals' wellness and engagement.
- Ensuring compliance with legislative requirements and upholding Aged Care Quality Standards, and other relevant Aged Care frameworks and guidelines.
- Managing and overseeing a portfolio of community-based Aged Care Participants, supporting them to remain living in their own homes in a safe, positive, comfortable way.
- Coordinate and oversee the implementation of care plans, ensuring services align with Participant needs and goals, preferences, and regulatory standards.
- Create & maintain accurate Participant records, documentation, budgets, and progress notes using CMS, ensuring regular review in line with organisational processes.
- Collaborating with other teams to ensure participant needs are met.
- Manage issues, mitigate risks, and identify potential risks, overall providing exceptional care to your participants.
- Daily assessment and actioning of case notes and incident reports.
- Visiting participants in their homes in accordance with organisational procedures and legislation requirements.
- Conduct comprehensive assessments of Participants to develop, review, and update individual care plans.
Requirements
- Able to work as part of a professional team and collaborate effectively with colleagues.
- Strong administrative skills and ability to build rapport with staff and participants.
- Knowledge of aged care sector desirable.
- Demonstrated ability to work co-operatively, autonomously, and with a high degree of initiative.
- Proficient in computer software including word, excel, internet, and email.
- Excellent written and verbal communication skills.
- Police check (less than 3 months).
- Current WWVP with NDIS endorsement (or ability to gain endorsement).
- Drivers license.
