Geriatric Medicine Clinic Coordinator Role

This is an exciting opportunity to join our team as a Geriatric Medicine Clinic Coordinator.

  • The successful candidate will lead the efficient delivery of high-quality, patient-centred Geriatric Medicine Clinics.

Key Responsibilities include:

  • Lead and coordinate the daily operational and clinical functions of the Geriatric Medicine Clinics.
  • Manage internal and external referrals to ensure a streamlined and responsive patient journey.
  • Apply clinical knowledge to triage and schedule referrals in collaboration with medical and administrative staff.
  • Engage directly with patients to support referral triage, connect them with primary care, and facilitate ongoing referrals to support services.
About You
  • You should have appropriately recognised tertiary qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA), e.g., Registered Nurse or Allied Health Professional.
  • Proven clinical expertise, ideally gained in public health environments and in delivering care to older populations.
  • A commitment to delivering patient-centred care, with a deep understanding of the unique needs of older adults.
  • Strong clinical assessment, triage, and coordination skills.
  • Demonstrated leadership capabilities and a passion for continuous service improvement.
  • Excellent communication, interpersonal, and stakeholder management skills.
  • Strong organisational and problem-solving abilities.
  • A commitment to delivering safe, high-quality care aligned with Western Health's values and the Live Best Care framework.
  • Proficiency in electronic medical records and referral management systems.
