**Job Overview**

We are seeking a Care Coordinator to join our team. In this role, you will be responsible for assisting in the development and delivery of social and lifestyle support activity programs for consumers.

**Key Responsibilities:**

  • Treat consumers with kindness, respect, and dignity.
  • Assist consumers in making day-to-day choices to live their lives so they have control over their lives.
  • Deliver lifestyle programs as planned, involving, engaging, and encouraging consumers to participate.
  • Document consumer outcomes clearly and as per procedures.
  • Recognize changes in a consumer's health status and report those appropriately.
  • Adhere to workplace safety laws and regulations.
  • Work cooperatively with colleagues and adhere to HR policies.

**Essential Qualifications & Requirements:**

A Certificate IV in Leisure and Health or similar qualification is highly desirable.

12 months experience working in Aged Care delivering social and recreational activities and programs to seniors.

A customer-centric approach and genuine passion for working in Aged Care.
