We are seeking highly motivated and dedicated individuals to join our team as Care Coordinators in Melbourne, Victoria.
As a Care Coordinator, you will play a vital role in providing high-quality support and care to participants with disabilities, enabling them to live independently and achieve their goals and aspirations.
You will have the opportunity to work in a supportive team environment, utilizing your skills and experience to make a real difference in people's lives.
Key Responsibilities- Provide high-quality support and care to participants with disabilities, including daily personal care and household activities
- Implement individualised plans to maximise participant outcomes and attainment of goals
- Maintain participant health and wellbeing, and follow individual care plans to support participants in enhancing their independence
- Unrestricted driver's licence in the state of Victoria
- Flexibility to work a range of shifts, including mornings, afternoons, evenings, and weekends
- Current first aid and medication certificates, or willingness to obtain
- Relevant qualification recognised by Possability
- Previous experience in a relevant industry, service, or equivalent level of expertise and experience
